# How to make physician call fair when nights, weekends and holidays aren’t equal

An even count of calls is not an even share when one partner keeps landing Saturdays and Christmas. Here is how physician groups weight call, set each person’s target, and keep it fair from one year to the next.

**Short answer** Give each kind of call a weight the group agrees on (a weeknight is 1 point, a weekend day more, a major holiday most). Set each physician a points target in proportion to their FTE and any relief the group grants. Then check every schedule against those targets, quarter by quarter and year over year. Keep holidays on their own rotation, count trades and payback in the same ledger, and show everyone the numbers.

## Why counting calls is not enough

Six physicians and 92 nights in a quarter: give everyone 15 or 16 calls and the count looks even. It isn’t, if one of them took five Saturdays and Thanksgiving while another took Tuesdays. A weekend costs a family day, a Friday night costs the weekend, and a major holiday costs the most. Groups that only count calls end up arguing about which calls.

The fix is old and simple: agree on what each kind of call is worth, give each person a target, and measure every schedule against it. The hard part is doing that every quarter, for every trade and every sick day, without a spreadsheet nobody trusts.

## Step 1: weight each kind of call

Some groups weight by workload (pages, admissions, cases overnight), others by how much people mind it. Keep it to a handful of categories everyone can recite.

## Step 2: set each physician’s target

- **By FTE.** A target is the quarter’s total points times the physician’s FTE, divided by the group’s total FTE. Many groups prorate call this way; others expect part-timers to take full call and pay them for the extra. Decide, and write it down.

- **Senior relief.** Many groups, and many hospital medical staff bylaws, excuse senior physicians from some call after a set age or years of service. It is especially common in obstetric and surgical groups. Decide whether relief lowers their target or moves it to the calls they still take.

- **A number the group sets.** A new partner taking extra call in year one, or a physician carrying a leadership role taking less.

## A worked example: one quarter, six physicians

October 1 to December 31, 2026 has 92 nights: 50 weeknights, 11 Fridays, 26 weekend days, 3 minor holidays (the day after Thanksgiving, Christmas Eve and New Year’s Eve) and 2 major ones (Thanksgiving and Christmas Day). At the weights above, that is 132 points. The group has 5.4 FTE, so a full-time physician’s target is 24.4 points. LM has senior relief: no weekends or holidays, with the same target, taken on weeknights.

Everyone lands within 0.6 points of their target. LM’s relief is visible too: no weekends or holidays, and 24 weeknights, nearly two a week. That is the trade the group agreed to, written where everyone can see it.

## Holidays need a memory

A single quarter can be perfectly balanced and still unfair across years. Keep holidays on their own rotation that carries over, not one that resets every January:

- **Tier them.** Major (Thanksgiving, Christmas, New Year’s Day) and minor (the Fourth of July, Labor Day, Memorial Day, Christmas Eve). Decide whether Christmas Eve travels with Christmas Day or is its own call.

- **Rotate by last year.** Whoever had Christmas last year doesn’t get it this year; whoever had it two years ago is next in line.

- **Count them twice.** Holiday points count toward the quarter’s target, and the holiday itself counts on the multi-year rotation.

## Trades, sold call and payback

- **Trades count at their weights.** A Saturday traded for a Tuesday is not even, and the ledger should say so.

- **Sold call.** Some groups let physicians buy and sell call for pay. The clean rule: points go to whoever worked the call, and the money settles the rest. Otherwise targets stop meaning anything.

- **Payback.** When someone is out sick or on leave, whoever covers is owed. Track owed calls and pay them back in the next schedules, instead of trusting everyone’s memory.

## Make the numbers visible

Most call disputes are about trust, not arithmetic. When every physician can see their own points against target, the group’s spread, the holiday rotation and who owes whom, the conversation moves from “I always get Saturdays” to “I am 1.5 points over, give me a lighter month.”

- **Weights and targets as settings.** By FTE, by relief, or a number you set, not formulas in a spreadsheet.

- **Balance every draft against targets.** Every rule checked, and everyone’s points close to target, before anyone sees it.

- **Remember holidays across years.** So next year’s Christmas goes to whoever is due.

- **Keep trades and payback in the ledger.** At their weights, with who owes whom carried forward.

- **Show everyone the counts.** On their phone, next to their schedule.

### How do you divide call fairly among physicians?

Agree on a weight for each kind of call (a weeknight, a Friday, a weekend day, a minor and a major holiday), give each physician a points target in proportion to their FTE and any relief the group grants, and balance every schedule against those targets. Keep holidays on their own rotation across years, and count trades and payback in the same ledger.

### Should part-time physicians take less call?

That is the group’s decision, and it should be written down. Many groups prorate call by FTE, so a 0.8 FTE physician carries 80% of a full share. Others expect part-timers to take full call and pay them for the extra. Either works if the target is explicit and the schedule is checked against it.

### How should weekends and holidays be weighted?

Pick a few categories everyone can remember and give each a weight: for example 1 point for a weeknight, 1.5 for a Friday night, 2 for a 24-hour weekend day, 2.5 for a minor holiday and 3 for a major one. The exact numbers matter less than agreeing on them and applying them to every schedule.

### Does traded or sold call count toward a physician’s share?

The cleanest rule is that points go to whoever worked the call, at its weight, and any money between physicians settles the rest. Then a Saturday traded for a Tuesday shows up as uneven, and every target still means what it says.
